MAHARASHTRA LABOUR LICENCE:- Application for Licence Form-IV (See rule 21(I)) Select your Division, District and Labour Office as per location of work 1) Name and complete address of the contractor (Please Furnish Firm name) 2) Email id and mobile number 3) Principal Employer Registration No 4) Name and complete Address of Establishment 5) Name and complete address of the Principal Employer (mobile no.) 6) Nature of work in Establishment 7) Nature of Process,Operation or Work in which Contract Labour is or is to be employed in the Establishment 8) Estimated work start date and end date 9) Name and complete address (local address) of the Manager of Contractor at work Establishment (mobile no.) 10) No. of contractor labour to be employed *Here establishment means the company who had given the contract and principal employer means any person responsible for the supervision and control of the said establishment. Requirements:- 1) Photo of Proprietor/ Authorized Partner/Director 2) Signature specimen on plain paper 3) Pan and Aadhaar card of Proprietor/ Authorized Partner/Director 4) Annexure-A 5) Annexure-B 6) Form IV-A form of undertaking 7) Form V – certificate from principal employer 8) Pan card of firm/company 9) Partnership deed/ Certificate of incorporation/ MOA/AOA 10) GST licence 11) Email id and mobile number 12) Authority Letter 13) Work order/contract 14) Principal Employer Registration number 15) Name, complete address (local address) and mobile no. of the Manager of Contractor at workplace. 16) No of contract labour employed 17) Deposit @ Rs. 500 per employee 18) Govt Filing fees Rs. 5000/- per year
